To determine if anemia, elevated platelet-lymphocyte ratio leukocytosis, elevated neutrophil-lymphocyte ratio, and leukocytosis are predictors of complications in patients with acute diverticulitis. Material and methods: An analytical, retrospective cohort study was carried out in which 244 patients with acute diverticulitis were included, according to selection criteria which were divided according to the presence or absence of elevated neutrophil lymphocyte ratio, elevated platelet/lymphocyte ratio, anemia and leukocytosis it was calculated the chi square and the relative risk statistician. Results: The mean age and the frequency of obesity and diabetes mellitus were significantly higher in patients with complicated acute diverticulitis (p<0.05), the elevated neutrophil/lymphocyte ratio as a predictor of complications in patients with acute diverticulitis with a relative risk of 1.91 (p<0.05); elevated platelet/lymphocyte ratio as a predictor of complications in patients with acute diverticulitis with a relative risk of 2.78, (p<0.05); leukocytosis as a predictor of complications in patients with acute diverticulitis with a relative risk of 2.37 (p<0.05); anemia as a predictor of complications in patients with acute diverticulitis with a relative risk of 2.01 (p<0.05); which were significant. In the multivariate analysis through logistic regression, the variables advanced age, obesity and diabetes mellitus were identified as risk factors for complicated acute diverticulitis. Conclusion: anemia, elevated platelet-lymphocyte ratio leukocytosis, elevated neutrophil-lymphocyte ratio, and leukocytosis are predictors of complications in patients with acute diverticulitis
