Abstract: Objetivo: Evaluar la validez de la escala ABC como pronóstico de mortalidad en pacientes con hemorragia digestiva alta.
Material y métodos: Se realizó un estudio observacional de pruebas diagnósticas. Se incluyeron 157 pacientes atendidos en el departamento de Emergencia de Medicina del Hospital Belén de Trujillo durante el periodo 2021-2022. Para cada participante, se calculó su puntaje en la Escala ABC, y su predicción de mortalidad. Se elaboraron tablas cruzadas, se construyó una Curva ROC y se calcularon los Indicadores Clínicos para el punto de corte óptimo. Las variables asociadas con la mortalidad fueron sometidas a un análisis bivariado y multivariado; se usó la Razón de Prevalencia como indicador de la fuerza de asociación.
Resultados La mortalidad se encontró en 15,2%. La Escala ABC demostró un Área bajo ROC de 0,980 (IC 95%: 0,961 – 1,000). Se identificó un punto de corte óptimo en un puntaje ≥ 8, donde obtuvo una Sensibilidad del 95,8%, Especificidad de 95,5%, Valor Predictivo Positivo de 79,3%, Valor Predictivo Negativo de 99,2%, Razón de Verosimilitud Positiva de 21,243 y Razón de Verosimilitud Negativa de 0,044. Las variables que mostraron asociación con la mortalidad fueron Puntuación ASA ≥ IV (RP: 44,597; IC 95%: 6,297 – 316,142; p=0,000) y Transfusión de Paquetes Globulares (RP: 2,999; IC 95%: 1,024 – 8,782; p=0,045).
Conclusión: Los resultados obtenidos muestran que la Escala ABC es válida como pronóstico de mortalidad en pacientes con hemorragia digestiva alta

A total of 157 patients attended in the Emergency Medicine Department of the Belén Hospital of Trujillo during the period 2021 2022 were included. For each participant, their ABC Score and predicted mortality were calculated. Cross tabulations were elaborated, a ROC curve was constructed and Clinical Indicators were calculated for the optimal cut off point. Variables associated with mortality were subjected to bivariate and multivariate analysis; the Prevalence Ratio was used as an indicator of the strength of association. Results: Mortality was found to be 15.2 %. The ABC Score showed an AUROC of 0.980 (95% CI 0.961 1.000).
