To analyze the association of BMI in the development of postsurgical complications in patients undergoing laparoscopic cholecystectomy. Material and methods: An analytical cross-sectional study was carried out on patients undergoing laparoscopic cholecystectomy at the Cayetano Heredia Hospital in Piura, 381 medical records were reviewed, leaving 139 that met the inclusion criteria for analysis. The exposure variable evaluated was the Body Mass Index and the primary outcome was analyzed: the association with postsurgical complications due to laparoscopic cholecystectomy, which were classified according to the Clavien Dindo classification, and as a secondary outcome: the hospital stay. Results: The most prevalent postsurgical complications were those classified as Clavien Dindo type I, with vomiting (11.5%) and seroma (6.5%) being the most frequent. Regarding Clavien Dindo type II and III complications, elevated pancreatic enzymes (2.9%) and residual common bile duct stones (2.2%), respectively, were the most frequent. Seroma and residual common bile duct stones were the two variables that were related to a BMI > 30, presenting a statistical significance of (p: 0.020) and (p: 0.019) respectively. On the other hand, vomiting (p: 0.01), surgical site infections (p: 0.045), seroma (p: 0.01), elevation of pancreatic enzymes (p: 0.000) and residual lithiasis (p: 0.000) were the complications associated with longer hospital stay. Conclusions: Only two complications after laparoscopic cholecystectomy (residual common bile duct stones, seroma) were associated with a BMI > 30 kg/m2. Furthermore, it was concluded that an obese female patient admitted for emergency is 7.6 times more likely to develop seroma as a postsurgical complication compared to patients without these characteristics
