Background: Acute otitis media (AOM) is a health problem that affects 80% of the pediatric population. The duration of antibiotic treatment is vital to avoid recurrences and bacterial resistance, however, several large-scale studies have controversial results. Objective: To compare the efficacy of antibiotic therapy according to the duration of treatment in infants with acute otitis media. Methods: A strategic search was carried out in 5 medical databases. All studies that satisfied the following criteria were incorporated: 1) Primary studies where the population is infants; 2) Controlled clinical trial type studies that compare a long course of antibiotic therapy against a short course; 3) Independent studies of date and language. The main outcome was to determine the effectiveness of the long course compared to the short course of antibiotic therapy for the treatment of infants with AOM. The risk of bias was analyzed through the RoB 2.0 program. The meta-analysis using the RevMan 4.5 program. The evaluation of the quality and certainty of evidence was carried out by the GRADEpro program. Results: Six randomized clinical trials were included with a total of 2,220 patients. The long course was greater by 14% compared to the short course of antibiotic therapy; (RR: 1.14; IC 95% 1.08 – 1.21; p < 0.00001). The most frequently reported adverse effects were diarrhoea, nausea and dermal. Conclusions: The efficacy of the long course of antibiotic therapy was greater compared to the short course of antibiotic therapy in the treatment of infants with AOM
