The objective of this study was to determine if positive fluid balance is a predictor of failure in ventilatory weaning in a pediatric ICU patient at Hospital Belén de Trujillo (BHT). Materials and methods: An observational study with retrospective cohort design was carried out in patients who were admitted under mechanical ventilation (MV) to the pediatric ICU of the BHT, in the period between January 2015 and December 2022. Clinical records were examined to obtain the water balance of the last 24 hours prior to weaning. Then it was recorded whether weaning was successful when the patient came off mechanical ventilation and was not reintubated within 48 hours. Results: 98 patients were included in the study. Of the patients with a positive fluid balance, 71.4% presented failure in ventilatory weaning; and of the total number of patients with negative fluid balance, 6.1% presented failure in ventilatory weaning. Positive fluid balance proved to be a predictor of failure in ventilatory weaning with statistical significance in x2 44.014 (degree of freedom 1, p < 0.001) and risk ratio (RR) 11.67 (CI 95% 3.84 - 35.42). Conclusions: Positive fluid balance is a predictor of failure for ventilatory weaning in patients admitted under mechanical ventilation to the pediatric ICU of BHT.
