To determine if the erythrocyte distribution width/serum albumin ratio is valid for the diagnosis of severe acute pancreatitis in patients at the Regional Teaching Hospital of Trujillo. Material and methods: A retrospective sectional study of diagnostic tests was carried out in which 244 patients with acute pancreatitis were included, according to selection criteria which were divided into 2 groups: severe and non-severe; calculating sensitivity, specificity, positive predictive value and negative predictive value. Results: The sensitivity and specificity of the erythrocyte distribution width/serum albumin ratio for the diagnosis of severe acute pancreatitis were 81% and 83%, respectively; The positive and negative predictive value of the erythrocyte distribution width/serum albumin ratio for the diagnosis of severe acute pancreatitis were 36% and 97%, respectively; The best cut-off point and the area under the curve of the erythrocyte distribution amplitude/serum albumin ratio for the diagnosis of severe acute pancreatitis were 4 and 79%, respectively. The average age and the frequencies of hyponatremia, hyperlactatemia, leukocytosis and diabetes mellitus were significantly higher in the severe acute pancreatitis group (p<0.05). Conclusion: the erythrocyte distribution amplitude/serum albumin ratio is valid for the diagnosis of severe acute pancreatitis in patients at the Regional Teaching Hospital of Trujillo
