To determine if the underweight is a risk factor for hepatotoxicity in patients with pulmonary tuberculosis. Material and Methods: The research is a prospective population cohort study with a population made up of 184 patients with pulmonary tuberculosis, of which 59 had underweight and 125 had no underweight in the city of Trujillo from 2015 to 2018. The risk was estimated using relative risk (RR) with a 95% confidence interval, establishing a significant association if p is less than 0,05. Results: The frequency of hepatotoxicity in patients with pulmonary tuberculosis who had underweight was 75% and the frequency of hepatotoxicity in patients with pulmonary tuberculosis who did not have underweight was 28%. A significant association was found between weight insufficiency (body mass index less than 18.5 kg / m2) and hepatotoxicity with a RR 2.66 (95% CI: 1.94 - 3.66). The average age of patients who have hepatotoxicity is 42.1 ± 21.6 and that of patients who do not have hepatotoxicity is 40.7 ± 20.1. Pearson's T - Test and Chi Square Test found no significant association for development hepatotoxicity at the 5% level (p > 0.05) for age (p 0.49), sex (p 0.238), HIV coinfection (p 0.255), diabetes mellitus (p 0.679), renal failure (p 0.609) and high blood pressure (0.554). Conclusion: Weight insufficiency is a risk factor for the development of hepatotoxicity in patients with pulmonary tuberculosis.
