Objective: To determine if nail psoriasis is a risk factor for the development of psoriatic arthropathy in adults aged between 20 and 70 years. Material and methods: An observational, analytical, retrospective cohort study population consisted of 189 patients according to established selection criteria divided into two groups: 63 patients with nail psoriasis (exposed) and 126 patients without nail psoriasis (unexposed), Chi-square of Pearson (x2) test was applied and Relative Risk (RR) was calculated. Results: The frequency of Psoriatic arthropathy in patients with nail psoriasis was 33/63 = 52,4%. The frequency of Psoriatic arthropathy in patients without nail psoriasis was 28/126 = 22,2%. Nail psoriasis is a risk factor for psoriatic arthropathy with a RR (IC 95%) of 2,357, which was significant (p<0,05). In the bivariate logistic regression analysis, it was found that there are no significant differences (p>0,05) in the variables studied, except for the severity variable that did find significance which could act as a confounding variable in the present study, however, fails to modify the relative risk (RR: 2,366) in an important way unlike the RR found between nail psoriasis and psoriatic arthropathy. This way, when nail psoriasis and severity are presented together, they represent a risk factor for psoriatic arthropathy. Conclusions: Nail psoriasis is a risk factor for the development of psoriatic arthropathy
