Objective: To demonstrate the association between hypoalbuminemia (albumin <3.5 g/dl) and postoperative morbidity in gastric cancer. Material and methods: An analytical, longitudinal, retrospective and case-control study nested in a cohort was carried out. 150 patients gastrectomized for gastric cancer at the Regional Institute of Neoplastic Diseases “DR. Luis Pinillos Ganoza” – IREN NORTE, in the period 2011-2023, among them, 50 patients gastrectomized for gastric cancer did develop postoperative complications and 100 patients gastrectomized for gastric cancer did not develop postoperative complications. Results: 42.6% of patients with hypoalbuminemia showed postoperative complications. Regarding intra-abdominal complications, anastomotic dehiscence and fistula (4.3%) and ISO (12.8%) were found. Likewise, with respect to extra abdominal complications, atelectasis (4.3%), pneumonia (12.8%) and sepsis (12.8%) were evident. It should be noted that, between the groups with and without hypoalbuminemia, regarding the factors studied, they did not have statistical significance, except in the anemia variable (p<0.05). In the bivariate analysis, the pathological stage was significantly associated with general (OR = 2.56; 95% CI = 1.04 – 6.30; p = 0.04) and intra-abdominal (OR = 1.14; 95% CI) complications. % = 1.02 – 1.26; p = 0.03); age >= 63 years was associated with general complications (OR = 2.36; 95% CI = 1.17 – 4.75; p = 0.02) and extra-abdominal complications (OR = 2.63; 95% CI = 1 .20 – 5.74; p = 0.01). Likewise, in the multivariate analysis, the pathological stage was significantly associated with intra-abdominal complications (OR = 15.08; 95% CI = 1.05 – 216.49; p = 0.04). Conclusion: Hypoalbuminemia is not a prognostic factor for postoperative morbidity in patients with gastric cancer
