To evaluate the capacity of the neutrophil/lymphocyte ratio (NLR) and the platelet/lymphocyte ratio (PLR) to predict complicated acute appendicitis in children. Methods: A diagnostic test study was conducted, including 270 pediatric patients treated at the Hospital Regional Docente de Trujillo between January 2016 and August 2023. Clinical and laboratory data were collected from medical records. The NLR and PLR ratios were calculated and a statistical analysis was performed, including the calculation of sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) for both ratios. Additionally, the area under the curve (AUC) was evaluated as a measure of diagnostic performance. Results: The mean age in the groups with and without complicated acute appendicitis was 11.12 + 3.87 years and 11.60 + 3.51 years respectively (p = 0.330); the proportion of males in the study groups was 64.48% and 56.32% respectively (p = 0.197). NLR was found to have a sensitivity of 77%, a specificity of 53%, a PPV of 77%, and a NPV of 52%. In contrast, the PLR had a sensitivity of 56%, a specificity of 57%, a PPV of 74%, and a NPV of 38%. The AUC for NLR was 0.732, while for PLR was 0.600. Conclusion: The NLR emerges as a better predictor than PLR for complicated acute appendicitis in children, showing higher sensitivity and better diagnostic performance in this patient group.
