Objective: To determine whether exclusive breastfeeding is a protective factor for skin allergies in preterm infants during the first year of life at Albrecht Hospital. Materials and Methods: Observational, analytical, retrospective case-control study, with a sample of 212 patients; 106 preterm patients with skin allergies and 106 preterm patients without skin allergies and their association with exclusive breastfeeding during the first year of life at the Albrecht Hospital. The data obtained were typed in a data collection sheet of the medical records provided from the database of the Albrecht Hospital of Trujillo during a period of 11 years, from the year 2012 to 2022. Subsequently, the information obtained was tabulated and processed using the IBM SPSS Statistcs 26.0 statistical software. Results: It was found that of the total number of preterm infants with skin allergies, 26.4% received exclusive breastfeeding (EBF); and of the total number of preterm infants without skin allergies, 84% received exclusive breastfeeding. Likewise, preterm infants who received exclusive breastfeeding had a 93% lower chance of having skin allergies compared to preterm infants who did not receive EML, being statistically significant [p<0.001; OR (95% CI): 0.07 (0.03 - 0.13)]. Additionally, the variable type of vaginal delivery was statistically significant [p<0.05; OR (95% CI): 0.492 (0.261 - 0.929) and was considered a protective factor; other variables such as family history of eczema were statistically significant, while the others were not. Conclusions: Exclusive breastfeeding is a protective factor for skin allergies in infants born prematurely during their first year of life
