Objective: To determine whether there is an association between the use of proton pump inhibitors (PPI) and the progression of chronic kidney disease (CKD). Methodology: We analyzed a retrospective cohort of 300 patients with stage III and IV CKD divided into two groups: those exposed to PPIs (minimum exposure time of 3 consecutive months) and those not exposed. In both groups it was determined whether or not they presented progression of CKD defined as a decrease in the glomerular filtration rate > 30% with respect to baseline, a doubling of the albumin/creatinine ratio, or a doubling of creatinine with respect to a previous value; in addition, it was related to intervening variables. Bivariate analysis was performed by calculating the relative risk (RR) with 95% confidence intervals, the association was considered significant if p<0.05. Results: 20 (17.2%) patients exposed to PPI and 37 (20.1%) patients not exposed to PPI presented progression of chronic kidney disease (RR: 1.17 CI 95%: 0.71-1.91; p= 0.53). The covariate related to CKD progression was age ≥ 65 years (RR: 1.81; 95% CI: 1.14-2.90; p=0.014). Conclusions: Proton pump inhibitor use was not associated with CKD progression
