Objective: To determine if the elevated urea / creatinine index (U/ Cr) is a predictor of acute kidney injury or death in decompensated heart failure (ICD). Materials and Methods: An analytical, observational, retrospective cohort study was carried out. In a sample of 186 patients hospitalized with ICD in the Department of Internal Medicine of the Hospital Belén in the city of Trujillo during the period 2013-2017, meeting the inclusion criteria and not presenting exclusion criteria. Non-probability sampling (for convenience) was used, selecting two groups: 93 with U / Cr index> 50 and 93 with U / Cr index ≤ 50. The information obtained was recorded on data collection sheets for subsequent bivariate and multivariable analysis in SPSS software as well as application of relative risk, chisquare statistical test for qualitative variables and logistic regression analysis. Results: It was found that there is a significant difference in relation to acute kidney injury (AKI) or death. The incidence of AKI was 68% in the group with U / Cr index> 50 and 12% in U / Cr ≤ 50. A relative risk of 5.73 was obtained (p <0.05). Likewise, the incidence of death was 17% in patients with U / Cr> 50 and 5% in U / Cr ≤ 50. A relative risk of 3.20 (p <0.05) was obtained. The logistic regression analysis corroborates the significance of the risk of glomerular filtration ≤ 60 and type 2 diabetes mellitus for AKI, while for death no statistical association was found with intervening variables (p> 0.05). Conclusions: The high urea / creatinine index is a predictor of acute kidney injury or death in decompensated heart failure.
