Objective: Determine if gestational diabetes is a risk factor for acute fetal distress in patients at the Belén Hospital in Trujillo. Material and methods: An analytical, retrospective cohort study was carried out in which 178 pregnant patients were included, according to selection criteria which were divided according to the presence or not of gestational diabetes, the chi square was calculated and the relative risk statistician. Results: The frequency of preeclampsia and obesity were significantly higher in the group of patients with gestational diabetes (p<0.05); the frequency of acute fetal distress in patients with gestational diabetes was 36%; the frequency of acute fetal distress in patients without gestational diabetes was 18%; Gestational diabetes is a risk factor for acute fetal distress with a relative risk of 2.06 which was significant (p<0.05); In the multivariate analysis, the variables gestational diabetes, preeclampsia and obesity were recognized as risk factors for acute fetal distress. Conclusion: gestational diabetes is a risk factor for acute fetal distress in patients at the Belén Hospital in Trujillo
