Objective: Determine the prognostic value of the metastatic lymph node ratio in the 5 years survival probability of patients with resectable colorectal carcinoma. Material and methods: The present observational, analytical and prospective research study, by means of a cohort type design; studied and analyzed the information of 63 patients with a diagnosis of resectable colorectal carcinoma treated at the EsSalud Hospital of High Complexity Virgen de la Puerta of Trujillo during the period from July 2015 to July 2018. Results: The average age of the total series was 64.24 ± 12.8 years (range: 37 - 89 years). The majority of the study population was female (54%), with a median time of illness of 06 months (range: 01 - 59 months). 57.1% of patients received treatment with adjuvant chemotherapy. The most frequent location of colorectal neoplasms was in the right colon (36.5%). The number of metastatic lymph nodes ranged from 0 - 16 (median: 0) and the total number of lymph nodes resected ranged from 12 - 40 (mean: 23.3 ± 8.2). There was no significant correlation between the number of metastatic lymph nodes and the number of lymph nodes excised (r = -0.073, p = 0.057). The global survival curve at 12, 36 and 60 months of the total series was 91.5%, 71.4% and 71.4% respectively. Survival at 5 years of patients with colorectal cancer according to N0, N1a, N1b, N2a and N2b were 76.6%, 85.7%, 0%, 75% and 50% respectively (p = 0.006). Survival at 5 years of patients with colorectal cancer according to LNR1 (0 - <0.06), LNR 2 (0.06 - 0.12), LNR 3 (> 0.12 - 0.24) and LNR 4 (≥ 0.25) was 75.6%, 58.3%, 75% and 50% respectively (p = 0.021). Conclusions: The metastatic lymph node ratio (LNR) is a much more complete 5-years survival predictor compared to the regional nodal status (N) of the TNM classification system of the American Joint Committee on Cancer (AJCC).
