Objective: Know the risk factors for severe leptospirosis in patients of hospitals two of departments of Peru. Material and methods: A case-control study was carried out, with a sample of 75 patients, 15 cases and 60 controls. Data were obtained from the clinical histories and epidemiological files of the patients who met the selection criteria, during the period of January 2011- December 2017, of II-2 Tarapoto Hospital, Belen Hospital of Trujillo and Regional Teaching Hospital of Trujillo. Results: Of the patients hospitalized with leptospirosis in the bivariate analysis we found that elevated serum urea (p = 0.001, OR 38.5, 95% CI 4.677- 316.861), severe anemia (p = 0.025, OR 5.091, 95% CI 1,103 - 23,493), leukocytosis (p = 0.006, OR 6, 95% CI 1,530-23,530), hypoxemia (p = 0, 001, OR 21,714, 95% CI 4,647 - 101,461), auscultation altered thoracic (p = 0, 001, OR 35.412, 95% CI 4.313 - 290.637), oliguria and anuria (p = 0.005, OR 14.75, 95% CI 1.411 - 154.158), vomiting (p = 0 , 001, OR 9,750, 95% CI 2.017 - 47.135) and another type of antibiotic different from the MINSA protocol (p = 0.009, OR 4, 571, 95% CI 1.343 - 15.109) were significant. In the multivariate analysis it was found that altered thoracic auscultation and vomiting were significant. In the logistic regression making a selection of variables using the Wald's forward method, the high serum urea, hypoxemia, altered thoracic auscultation and vomiting were significantly associated with severe leptospirosis Conclusions: Elevate serum urea, hypoxemia, altered thoracic auscultation and vomiting are predictors of severity in patients with leptospirosis in hospital in the departments of La Libertad and San Martin. It is necessary for the diagnosis and treatment of leptospirosis before it progresses to its severe form
