Abstract: Determinar si la edad, el sexo, el índice de masa corporal, la anemia, la
hipertensión arterial, la diabetes mellitus tipo 2, la enfermedad pulmonar obstructiva
crónica, el accidente cerebrovascular previo, la insuficiencia renal crónica, el trastorno
mental, el tiempo de espera, el tipo de fractura, el estado funcional, el uso de
psicofármacos y el grado ASA son factores de riesgo asociados a neumonía
intrahospitalaria en pacientes con fractura de cadera en el hospital Víctor Lazarte
Echegaray durante el periodo enero 2013 – abril 2019.
MATERIALES Y MÉTODOS: Se realizó un estudio observacional, analítico,
longitudinal, de casos y controles, donde participaron 160 pacientes con fractura de
cadera divididos en 2 grupos: un grupo de casos conformados por 32 pacientes con
neumonía intrahospitalaria y un grupo control conformado por 128 pacientes sin
neumonía intrahospitalaria. Asimismo, se realizó el análisis bivariado y multivariado
correspondiente para la obtención de resultados.
RESULTADOS: El análisis bivariado de los factores de riesgo estudiados evidenció
que, la edad (p<0,001), el tiempo de espera (p<0,001), la anemia (p=0,009), EPOC
(p=0,030), trastorno mental (p<0,001), estado funcional (p<0,001), uso de
psicofármacos (p<0,001) y el grado ASA (p<0,001) fueron estadísticamente
significativos. Con estos datos, se realizó el análisis multivariado donde se
mantuvieron como factores de riesgo significativos: la edad (OR=1,144; IC=1,050–
1,247; p = 0,002) y el tiempo de espera (OR=1,442; IC=1,266 – 1,644; p <0,001).
CONCLUSIONES: La edad y el tiempo de espera son factores de riesgo asociados
a neumonía intrahospitalaria en pacientes con fractura de cadera.
To determine whether age, sex, body mass index, anemia, arterial hypertension, type 2 diabetes mellitus, chronic obstructive pulmonary disease, previous stroke, chronic renal failure, mental disorder, waiting time, type of fracture, functional status, use of psychotropic drugs waiting time, type of fracture, functional status, use of psychotropic drugs and ASA grade are risk factors associated with nosocomial pneumonia in hip fracture patients at the Victor Lazarte Echegaray hospital during the period January 2013 - April 2019. MATERIALS AND METHODS: An observational, analytical, longitudinal, case-control study was conducted, involving 160 patients with hip fracture divided into 2 groups: a case group consisting of 32 patients with nosocomial pneumonia and a control group consisting of 128 patients without nosocomial pneumonia. RESULTS: The bivariate analysis of the risk factors studied showed that, age (p<0.001), waiting time (p<0.001), anemia (p = 0.009), COPD (p = 0.030), mental disorder (p<0.001), functional status (p<0.001), use of psychotropic drugs (p<0.001) and the ASA grade (p<0.001) were statistically significant. With these data, the multivariate analysis was carried out where the following were maintained as specific risk factors: age (OR = 1.144; CI = 1.050 – 1.247; p = 0.002) and waiting time (OR = 1.442; CI = 1.266 – 1.644; p<0.001).
