Abstract: Se han descrito complicaciones especificas asociadas con la cirugía de
cataratas, una de las que genera mayor impacto en costo sanitario e
individual en estos pacientes es la endoftalmitis postoperatoria. Esta
complicación ha disminuido en incidencia en la última década, sin embargo,
continúa dejando consecuencias devastadoras. En esta revisión
bibliográfica, se presentan las generalidades de cirugía de cataratas, la
incidencia, etiología, factores de riesgo, fisiopatología, clasificación,
manifestaciones clínicas, pruebas diagnósticas, prevención y manejo de la
endoftalmitis postoperatoria a cirugía de cataratas. Además, se hace
comparaciones entre la etiología descrita en los libros versus la etiología
evidenciada en diferentes estudios retrospectivos actuales. Asimismo, se
aborda la relación entre los gérmenes formadores de biofilm con la
endoftalmitis. También se hace referencia a las diferencias entre
endoftalmitis y el Síndrome toxico del segmento anterior. Un tema muy
discutido de manera global son los regímenes profilácticos, se hace una
reseña acerca del uso profiláctico de povidona iodada preoperatoria, también
del uso de la inyección intracameral de cefuroxima, moxifloxacino y
vancomicina, resaltando la relación de esta última con la vasculitis retiniana
oclusiva hemorrágica. Esta revisión tiene una metodología de tipo analítico,
con un enfoque cualitativo, la búsqueda literaria se realizó en diferentes
bases de datos como MEDLINE, PubMED, LILACS, Scielo, Scopus, EBSCO
y Science Direct. En el análisis se evidencia que lo más importante de la
endoftalmitis postoperatoria es evitarla, pero no existe profilaxis ni manejo
globalizado y esto puede ser secundario a que no existe una formulación comercial aprobada en América Latina, generando mayor probabilidad en
errores de dosificación, esterilidad, dilución o diluyente. La conclusión
principal enfatiza que tanto el manejo como la profilaxis se basa en la
correcta toma de decisiones de cada médico, dejando todo en el criterio
personal, ya que ellos deben tomar en cuenta la etiología más frecuente de
su zona, características de cada paciente, medicamentos disponibles, entre
otros. La literatura disponible hasta el momento muestra una disminución
muy notable en el último siglo en la incidencia de endoftalmitis
postoperatoria, además la etiología mostrada en los estudios más recientes
muestra mucha similitud con la literatura y a esto se le agrega los nuevos
estudios de los microorganismos formadores de biofilm.
Specific complications associated with cataract surgery have been described, one of which generates the greatest impact on healthcare and individual costs in these patients is postoperative endophthalmitis. This complication has decreased in incidence in the last decade, however, it continues to leave devastating consequences. In this bibliographic review, the generalities of cataract surgery, incidence, etiology, risk factors, classification, clinical manifestations, diagnostic tests, prevention and management of postoperative endophthalmitis after cataract surgery are presented. In addition, comparisons are made between the etiology described in the books versus the etiology evidenced in different current retrospective studies. Likewise, the relationship between biofilm-forming germs with endophthalmitis is addressed. Reference is also made to the differences between endophthalmitis and Toxic Anterior Segment Syndrome. A topic that is widely discussed globally is prophylactic regimens, a review is made about the prophylactic use of preoperative povidone iodine, also the use of intracameral injection of cefuroxime, moxifloxacin and vancomycin, highlighting the relationship of the latter with occlusive retinal vasculitis hemorrhagic. This review has an analytical methodology, with a qualitative approach, the literary search was carried out in different databases such as MEDLINE, PubMED, LILACS, Scielo, Scopus, EBSCO and Science Direct. The analysis shows that the most important thing about postoperative endophthalmitis is to avoid it, but there is no prophylaxis or globalized management and this may be secondary to the fact that there is no commercial formulation approved in Latin America, generating a greater probability of dosage errors, sterility, dilution or diluent. The main conclusion emphasizes that both the management and the prophylaxis are based on the correct decision making of each doctor, leaving everything to the personal criteria, since they must take into account the most frequent etiology in their area, characteristics of each patient, medications available, among others. The literature available to date shows a very notable decrease in the incidence of postoperative endophthalmitis in the last century, in addition the etiology shown in the most recent studies shows a lot of similarity with the literature and to this is added the new studies of the biofilm.
