Specific complications associated with cataract surgery have been described, one of which generates the greatest impact on healthcare and individual costs in these patients is postoperative endophthalmitis. This complication has decreased in incidence in the last decade, however, it continues to leave devastating consequences. In this bibliographic review, the generalities of cataract surgery, incidence, etiology, risk factors, classification, clinical manifestations, diagnostic tests, prevention and management of postoperative endophthalmitis after cataract surgery are presented. In addition, comparisons are made between the etiology described in the books versus the etiology evidenced in different current retrospective studies. Likewise, the relationship between biofilm-forming germs with endophthalmitis is addressed. Reference is also made to the differences between endophthalmitis and Toxic Anterior Segment Syndrome. A topic that is widely discussed globally is prophylactic regimens, a review is made about the prophylactic use of preoperative povidone iodine, also the use of intracameral injection of cefuroxime, moxifloxacin and vancomycin, highlighting the relationship of the latter with occlusive retinal vasculitis hemorrhagic. This review has an analytical methodology, with a qualitative approach, the literary search was carried out in different databases such as MEDLINE, PubMED, LILACS, Scielo, Scopus, EBSCO and Science Direct. The analysis shows that the most important thing about postoperative endophthalmitis is to avoid it, but there is no prophylaxis or globalized management and this may be secondary to the fact that there is no commercial formulation approved in Latin America, generating a greater probability of dosage errors, sterility, dilution or diluent. The main conclusion emphasizes that both the management and the prophylaxis are based on the correct decision making of each doctor, leaving everything to the personal criteria, since they must take into account the most frequent etiology in their area, characteristics of each patient, medications available, among others. The literature available to date shows a very notable decrease in the incidence of postoperative endophthalmitis in the last century, in addition the etiology shown in the most recent studies shows a lot of similarity with the literature and to this is added the new studies of the biofilm.
