Objective: To determine the relationship between the prevalence of early childhood caries according to the ICDAS II method with salivary pH, Trujillo - 2023. Materials and methods: This observational, comparative, cross-sectional study was carried out between July and August 2023 and included a total of 130 participants (72 children in the public educational institution and 58 in the private institution) aged 3 to 5 years, who were enrolled in the Educational Institution 81025 José Antonio Encinas or in the Private Educational Institution Cuna Jardin Materno Infantil California in the district of Víctor Larco Herrera, Trujillo. To determine the intra- and inter-rater reliability of the clinical measurement, a pilot study was carried out with ten participants, resulting in Kappa values above 0.85. To analyse the results, the Chi-square test was used with a significance level of 5%. Results: There was a relationship between the prevalence of early childhood caries according to the ICDAS II method and the level of salivary pH in infants of both institutions, a significance was obtained (p=0. 000), in addition, a greater predilection of acid pH and carious lesions was found in the male group (63%), there was a relationship with the age of the infant where the age group of 5 years was the one that showed the greatest number of cases (76.4%), according to the sector of the educational institution, a greater number of affected population was found in the private educational institution Cuna Jardín Materno Infantil California (87.9%). Conclusion: The prevalence of early childhood caries according to the ICDAS II method and the salivary pH level are related, mainly in the female sex, with more predilection for the 5-year-old age group, with a high frequency in the private sector institution
