Objective: To determine whether the presence of type 2 diabetes mellitus is a risk factor for postoperative complications in adult patients with acute cholecystitis. Materials and methods: An observational, retrospective cohort study was carried out to evaluate two groups of 65 patients with acute cholecystitis, one of them with type 2 diabetes mellitus and the other without this disease; all of them attended in the general surgery service of the Hospital Regional Docente de Trujillo during the period from January 2018 to June 2023. Results: Patients with DM2 presented a similar mean age (62.55 ± 16.99) to those without DM2 (56.52 ± 18.41), with a similar gender distribution. Patients with DM2 had a higher percentage of moderate cholecystitis compared to non- diabetics (38.46% vs. 16.92%, respectively). In addition, diabetic patients showed a higher prevalence of arterial hypertension (41.54% vs. 29.23%). Patients with DM2 had a higher proportion of postoperative complications overall compared to non-diabetics (49.23% vs. 15.38%), respectively. Furthermore, there was a tendency for surgery-related complications to be more frequent in patients with DM2 (21.54% vs. 9.23%). However, no significant differences were found in terms of medical complications. Regarding nosocomial infections, there was a slightly higher proportion in the group with DM2 (18.46%%% vs. 4.62%). Conclusions: Patients with acute cholecystitis presenting with type 2 diabetes mellitus have a higher proportion of postoperative complications overall and likewise, a higher proportion of surgery-related complications and nosocomial infectious complications
