Objective: To evaluate and compare the efficacy of percutaneous aspiration with sclerotherapy versus laparoscopic unroofing in the treatment symptomatic simple hepatic cysts. Methods: A systematic review of five databases was performed and retrospective cohorts were included. The effectiveness of percutaneous aspiration was compared with sclerotherapy versus laparoscopic unroofing in patients with symptomatic simple hepatic cysts. The ROBINS-I tool was used to assess the risk of bias of the studies. In addition, it was necessary to use the Review Manager 5.4 program to prepare the Forest Plot graphs, using the Relative Risk (RR) for dichotomous outcomes and the mean difference (MD) with standard deviation (SD) for continuous outcomes. Results: We included five cohorts with a total of 59 patients treated with percutaneous aspiration with sclerotherapy, compared with 81 patients who received laparoscopic unroofing. Regarding recurrence, no significant differences were found between both treatments (RR= 1.38, IC 95% [0.30 – 6.24], p=0.68 I²: 82%). Similarly, patients undergoing percutaneous sclerotherapy compared to patients undergoing laparoscopic unroofing, there were not differences in term of mortality reduction (RR= 1.10, IC 95% [0.15 – 7.79], p=0.40 I²: 0%). Finally, we demonstrate that there were not significant difference between both techniques in terms of overall complications (RR= 0.96, IC 95% [0.31 – 3.03], p=0.57 I²: 0%). Conclusion: The present meta-analysis demonstrates that there are not significant differences in recurrence, death, and overall complications between percutaneous aspiration with sclerotherapy and laparoscopic unroofing in patients with symptomatic simple hepatic cysts
