To determine the maternal-fetal risk factors associated with episiotomy in pregnant women at Belén Hospital in Trujillo. Materials and Methods: A case-control study was conducted using 213 medical records of pregnant patients treated at the Obstetrics and Gynecology Service of Belén Hospital Trujillo from 2017 to 2022. Cases included pregnant women who underwent an episiotomy, and controls were those who did not. The factors studied included maternal age, obesity, nulliparity, inadequate prenatal care, fetal macrosomia, presentation dystocia, and funicular dystocia. The statistical analysis calculated the odds ratio (OR) for each factor with 95% confidence intervals, with significance determined at p < 0.05. Results: Out of the 213 pregnant women included in the study, 53 underwent an episiotomy, while 160 did not. The risk factors that showed an association were maternal age < 20 years (OR: 24.73; 95% CI: 4.89-124.89), age 20 to 35 years (OR: 5.17; 95% CI: 1.18-22.62), nulliparity (OR: 19.06; 95% CI: 8.79 - 41.34), and obesity (OR: 0.48; 95% CI: 0.24 - 0.96). In the logistic regression analysis, nulliparity was the only factor that remained significantly associated. Conclusion: The risk factors for episiotomy in pregnant women were younger maternal age and nulliparity, while obesity was a protective factor.
