Valor diagnóstico de la prueba de flicker en el diagnóstico precoz de glaucoma

Abstract

Objetivo: Determinar el valor diagnóstico de la prueba de Flicker en el diagnóstico precoz de glaucoma. Método: El diseño fue descriptivo de pruebas diagnósticas; Se estudiaron: 38 adultos con sospecha de glaucoma y con resultados de Prueba de Flicker y tomografía en el Hospital Belén de Trujillo y el Instituto Regional de Oftalmología. Las medidas principales del desenlace fueron: sensibilidad, especificidad, valor predictivo positivo y negativo y el área bajo la curva ROC así como el índice Youden. Resultados : El punto de corte de la Prueba de Flicker de 2.8 es el que ofrece mayor valor en el diagnóstico sensibilidad 83% , especificidad 73%, valor predictivo positivo 37% y valor predictivo negativo 95.9%, el área bajo la curva ROC fue de 83% (.p<0.01) y el índice de Youden fue de 0.57(0.33- 0.80) Conclusión: La prueba de Flicker es una buena prueba con valor diagnóstico intermedio y valor predictivo negativo alto, se recomienda como prueba de detección temprana en glaucoma primario de ángulo abierto.

Objective: To determine validity diagnostic of Flicker test in the diagnosis of early open-angle glaucoma (OAG). Method: The design: was descriptive of diagnostic test; The setting is Belen’ Hospital Trujillo and Regional Institute of Ophtalmology. We studied : 38 patient with spectral domain optical coherence tomography (OCT) and Flicker test. Main outcome measures were the sensitivity, specificity, positive and negative predictive value and area under the receiver operating characteristic curve. Results: The Flicker of 2.8 test cut-off point is offering greater value in the diagnostic sensitivity 83%, specificity 73%, predictive value positive 37% and predictive value negative 95.9%...The area under the ROC curve was 83% this is a predictive accuracy intermediate (.p<0.01).The Youden index was 0.57 (0.33 - 0.80) of efficiency diagnostic intermediate. Conclusions: Flicker’ Test is a good test has an intermediate diagnostic value and a high negative predictive,it is recommended as evidence of early detection in primary open angle glaucoma
