To determine whether the neutrophil/lymphocyte index is a predictor of mortality in patients starting hemodialysis. Materials and Methods: An observational, analytical, retrospective cohort study of 158 patients with chronic kidney disease who started hemodialysis between 2015 - 2019 at the Victor Lazarte Echegaray - Trujillo Hospital was performed. Patients were divided into two groups according to their neutrophil/lymphocyte index ≥ 3.5 or < 3.5. The relative risk of mortality was calculated according to the neutrophil/lymphocyte index and related to the intervening variables. Results: Of 158 patients, 72 (45.5%) had a neutrophil/lymphocyte index ≥ 3.5 of whom 28 (38.9%) died, of the 86 (54.4%) patients with index < 3.5, 36 (41.9%). died. The relative risk of mortality according to neutrophil/lymphocyte index ≥ 3.5 was 0.93 (CI95% 0.63 - 1.36) (p=0.7047). Multivariate analysis found that the only variable associated with mortality was age over 60 years old (p=0.0192). Conclusions: The neutrophil/lymphocyte index does not predict mortality in patients with chronic renal disease who start hemodialysis
