Objectives: The main objective of the study is to estimate the underdiagnosis of infectious mononucleosis in Peru. In addition, review the basic aspects of the disease, its etiology and proper diagnosis, and compile cases where the underdiagnosis of infectious mononucleosis brought complications to the patient's health. Procedures: The articles published in the last 10 years in the Pubmed, ClinicalKey and Google academic databases found were reviewed using the keywords: “infectious mononucleosis”, “diagnostic”, “case report”, “complications”. The articles were selected according to the criteria of the research team. Main findings: Infectious mononucleosis is a clinical syndrome that is caused by multiple agents, mainly the Epstein Barr virus, and that is characterized by fever, lymphadenopathy and pharyngitis associated with atypical mononuclear cells and lymphocytosis. In Peru there may be an underdiagnosis of this pathology as reported by the latest publications of the National Institute of Health in contrast to the reality reported in studies from other countries. This syndrome, generally benign, can bring with it serious problems for the patient's health, ranging from fulminant cases to serious complications. Conclusions: According to the review, it is deduced that infectious mononucleosis is an underdiagnosed syndrome in Peru. Complications collected show that infectious mononucleosis is not always a benign pathology.
