Objective: Identify the risk factors associated with postoperative osteomyelitis in patients with femur fracture treated at the Victor Lazarte Echegaray Hospital (HVLE) in Trujillo. Material and methods: A case-control, cross-sectional analysis study was carried out in 106 patients. Double-entry frequency tables, absolute values, central means and percentages of categorical variables were designed. For the bivariate and multivariate analysis, the Prevalence Ratio (aPR) with a 95% confidence interval with a significance of 5% was used. Results: The prevalence of postoperative osteomyelitis was 58.5%, according to the Cierny-Mader classification, 35.8% were in grade I. In the surgical process, blood transfusion and surgical vacuum time were significantly associated, and in the multivariate analysis, sex (p = 0.03), BMI condition (p = 0.02), diabetes mellitus (p = 0.01), open fracture (p = 0.01) and degree of AO classification (p = 0.04) were associated with osteomyelitis. The ROC Curve showed that diabetes mellitus and open fracture are predictors of postoperative osteomyelitis due to femur fracture in 79.5%. Conclusion: Diabetes mellitus, elevated BMI, and open fracture are highly predictive for developing postoperative osteomyelitis
