The research work aimed to determine if elevated HDL values are a risk factor associated with in-hospital mortality in patients with acute coronary syndrome at the Víctor Lazarte Echegaray Hospital, during the period between 2015 - 2022. Materials and methods: To carry out the research work, an observational, analytical, retrospective cohort study was carried out. In which we consider as a population the patients with a diagnosis of acute coronary syndrome who have been requested HDL values, for this a population made up of 98 patients was needed, both in the exposure group and in the non-exposure group, being a total of 196 patients included in our study Results: There were 196 patients with acute coronary syndrome who were followed up until their hospital discharge, of which 181 survived and 15 died. A statistically significant association was found between the female sex (RR: 3.52 IC95%1.25 - 9.92, p=0.017), and type 2 diabetes mellitus (RR: 4.05 IC95%1.51 - 10.85, p=0.005) and the increased risk of mortality. in patients with acute coronary syndrome. High HDL values did not present a statistically significant association (RR: 0.88 95% CI 0.33 - 2.33, p =0.789). A multivariate analysis was carried out, with the significant variables and a statistically significant association was again presented, type 2 diabetes mellitus increases the risk of death in patients with SICA by 20% (RR: 1.2 95% CI 0.15 - 2.25, p= 0.025). Conclusions: The study concluded that high HDL level is not associated with increased in-hospital mortality in patients with acute coronary syndrome, but type 2 diabetes mellitus is.
