Objective: To determine if hyperlactacidemia and metabolic acidosis are prognostic factors of mortality in patients with severe acute pancreatitis treated at Belen Hospital in Trujillo. Material and Methods: An analytical, observational, retrospective study of cases and controls was carried out. The study population consisted of 114 patients with severe acute pancreatitis according to established inclusion and exclusion criteria divided into two groups: deceased or survivors, the odds ratio and chi square test were calculated. Results:The average age and the frequency of hemoconcentration and hyperglycemia was significantly higher in the group of patients with severe acute pancreatitis who died than in the survivors. Hyperlactacidemia is a prognostic factor in mortality in patients with severe acute pancreatitis with an odds ratio of 2.30 which was significant (p <.005). Metabolic acidosis is a prognostic factor of mortality in patients with severe acute pancreatitis with an odds ratio of 2.44 which was significant (p <.005). In the multivariate analysis, the significance of the risk was verified for the variables hyperlactacidemia, metabolic acidosis; age over 60 years, hemoconcentration and hyperglycemia as prognostic factors of mortality in patients with severe acute pancreatitis. Conclusions: Hyperlactacidemia and metabolic acidosis are prognostic factors of mortality in patients with severe acute pancreatitis treated at Belen Hospital in Trujillo.
