Objective: Determine the effectiveness of Timolol-Dorzolamide-Brimonidine compared to Timolol-Dorzolamide in the decrease of intraocular pressure. Materials and Methods: A retrospective cohort study was conducted, evaluating patients diagnosed with Primary Open Angle Glaucoma treated at the Regional Institute of Ophthalmology between August 2015 and December 2016, randomly selecting 72 patients treated with Timolol-Dorzolamide-Brimonidine and 72 patients treated with Timolol- Dorzolamide; the patients were followed for 1 month. We used to Chi square test for qualitative variables and t-student for quantitative variables, it was established as statistical significance at p <0.05. Results: 280 eyes studied, which 139 eyes received Timolol-Dorzolamide-Brimonidine (TDB) and 141 eyes received Timolol-Dorzolamide (TD) for 1 month. The percentage of average reduction was greater for TDB obtaining in OD 31.62% and OI 31.43% compared to TD that obtained in OD 24.44% and OI 25.44%, with a statistically significant association OD p <0.001 and OI p = 0.005. Likewise, tolerability in both groups of patients showed to be statistically similar p = 0.630, however TDB obtained more fluctuations during the day (41.67%) compared to TD (20.83%) p = 0.007. Finally, the number of responders was higher in the TDB group obtaining a total of 122 (87.05%) eyes responders compared to TD that obtained 98 (69.50%) eyes responders p <0.009. Conclusion: Both fixed combinations managed to satisfactorily lower IOP and have good tolerability to the drug, but the TDB group was the one that showed the greatest effectiveness in reducing IOP, however it was the one that presented the most cases of fluctuations during the study.
