Objective: To establish the validity of the Red Blood Bell Distribution Width (RDW) compared with the BISAP score (Bedside Index for Severity in Acute Pancreatitis) to predict organic failure due to acute pancreatitis. Material And Methods: Analytical, cross-sectional study of diagnostic tests That counted with 102 Participants of the Hospital Víctor Lazarte Echegaray of Trujillo - Peru from the 2013-2015 Period, randomly chosen under ad hoc statistical sampling of a population of 126 patients diagnosed with acute pancreatitis, who met the selection criteria. To establish the optimal cut-off point of the ADE by receiver operator curves and compare the area under the curve, Sensitivity, specificity, positive predictive value, and negative predictive value, with the BISAP score to predict organic failure by acute pancreatitis, thus evaluating its validity Results: The cut-off point for ADE was 14.25%. Sensitivity, specificity, positive predictive value, and negative predictive value were 69.23%, 93.42%, 78.26%, and 89.87% for ADE; and 100%, 98.68%, 96.3%, and 100% for BISAP, respectively. The area under the curve for ADE and BISAP was 0.826 (95% CI 0.715 to 0.938) and 0.999 (95% CI 0.996 to 1), respectively (p = 0.0025). The ADE predicted admission to the Intensive Care Unit with a sensitivity, specificity, positive predictive value and negative predictive value of 64.29%, 84.09%, and 39.13%. Conclusions: The optimal cut-off point of the Red Blood Bell Distribution Width is useful for predicting organic failure due to acute pancreatitis. This cut-off point has a lot to predict UCI admission. However, the BISAP score continues to have greater precision to predict these two outcomes.
