OBJECTIVE: To determine clinical efficacy and safety of nebulization with 3% saline compared with 0.9% saline in infants with bronchiolitis. MATERIAL AND METHOD: prospective, randomized, blind study of infants between 2 and 24 months, hospitalized, divided into 02 groups of 63 patients each, assigned to nebulize with SS 3% or SS 0.9% every 6 hours until discharge. Clinical efficacy was measured with oxygen saturation, Bierman and Pierson score and hospital stay; the security registering the collateral effects. RESULTS: the average age was 6.29 months (SSH) and 6.46 months (SSN) with predominance of the male sex in both and an average saturation of 91.4% (SSH) 91.1% (SSN) at admission. At 48 hours 100% of patients in both groups had> 90% saturation (95% SSH vs 94.5% SSN) (p> 0.005). The clinical score remained moderate in both groups until 48 hours (5.3 SSH vs. 5.6 SSN), only 72 hours after mild (4.4 SSH vs 4.6 SSN), in all cases the differences were not significant. 61.9% of patients in the SSH group and 50.8% in SSN had a 72-hour stay (p> 0.05). In 2 cases (SSH), vomiting was reported as side effects. CONCLUSIONS: SSH compared to SSN has not been found to significantly influence oxygen saturation, pulse oximetry or hospital stay.
