To demonstrate that the Clobetasol 0.05 % cream is more effective than 0.1% tacrolimus for the treatment of localized alopecia areata. Material and Methods: Prospective, longitudinal study with a control group. The sample was initially composed of 97 patients from the Dermatology Essalud IV Hospital Victor Lazarte Echegaray Trujillo, of which 88 met the criteria for inclusion and exclusion, under randomized probability sampling and divided into two groups: 44 cases for the experimental group Clobetasol was applied to cream and 44 to control group Tacrolimus ointment. Control patients was 12 weeks to determine the percentage recovery of hair and compare the efficacy of both drugs. Results: the two groups, experimental and control, with 44 patients each were evaluated. The age was 33.23 ± 11.27 and 36.64 ± 12.98 for groups of Clobetasol and Tacrolimus respectively. In the first, the male had the 56.82% of cases, while in the second the female was 54.55%. The average percentage of hair loss in the experimental group decreased from 25.09 to 11.95 in 12 weeks of control over the decrease in the control group 22.2 to 11.05. The effectiveness of the recovery Clobetasol hair had an average of 54.95 ± 19.50, with a range of 16-100%, 5 cases and no adverse reactions in 88.6%. The efficacy of Tacrolimus in the recovery of hair had an average of 48.57 ± 13.25, range 5-74%, with 4 cases of adverse reactions and none in 90.9%. Comparison of efficacy for hair recovery showed a mean difference = 7.57, t = 1.795, p = 0.077. Comparison of efficacy according adverse reactions had a chi2 = 0.124, p = 0.725, OR = 1.282, 95% CI = 0320-5131. Conclusions: The effectiveness of Clobetasol versus Tacrolimus is similar as hair restoration and adverse reactions. The choice of drug should be at the discretion of the treating physician based on experience and updating.
