Abstract: Objetivo: Delimitar si el Índice Child Pugh + Lactato tiene una sensibilidad mayor al
80% como predictor de muerte en pacientes con cirrosis hepática hospitalizados en
el servicio de Gastroenterología del Hospital Regional Docente de Trujillo, 2012 –
2017.
Material y Método: Se llevó a cabo un estudio de prueba diagnóstica, retrospectivo
y observacional en una población de 121 pacientes diagnosticados con cirrosis
hepática y hospitalizados en el servicio de Gastroenterología del Hospital Regional
Docente de Trujillo durante el periodo Enero 2012 – Enero 2017. Se calculó la
sensibilidad, especificidad, valores predictivos, exactitud diagnóstica; así como el
punto de corte de dicho índice. Se aplicó el test de Chi cuadrado, T- Student y Área
bajo la curva.
Resultados: De los 121 pacientes incluidos en el estudio, fallecieron un total de 40
(33%) y sobrevivieron 81 (67%). El punto de corte del índice Child Pugh + Lactato
como predictor de muerte en pacientes con cirrosis hepática fue ≥ 11 puntos; con
una sensibilidad, especificidad, valor predictivo positivo y negativo de 87,5%, 49,4%,
46,1% y 88,9% respectivamente. La exactitud diagnóstica del mismo fue de 70,5%.
El promedio del índice Child Pugh + Lactato fue mayor en el grupo de pacientes
cirróticos fallecidos, este presentó significancia estadística (p=0.002).
Conclusiones: El Índice Child Pugh + Lactato no posee valor como predictor de
muerte en pacientes con cirrosis hepática.
