Sobrevida libre de enfermedad de pacientes con cáncer de mama con relación HERB2 positivo en el HMC periodo 2006 - 2014

Abstract

Objetivo: Precisar la sobrevida libre de enfermedad de pacientes con diagnóstico anatomopatológico de carcinoma ductal infiltrante Herb2/neu (+) con RE (-) y Luminal A con estadio clínico III o IV que fueron atendidos en el servicio de Oncohematología del Hospital Militar Central entre los años 2006 a 2014.

Material y Métodos: Se llevó a cabo un estudio de tipo descriptivo, analítico, retrospectivo como estudio de supervivencia. La población de estudio está constituida por 38 pacientes asignados en dos grupos, uno con Her2/neu (+) con RE (-) con estadio clínico III o IV (n=20) y el otro con Luminal A (n=18) en estadio clínico III o IV con carcinoma ductal infiltrante de mama en el servicio de Oncohematologia del Hospital Militar Central de Lima periodo 2006-2014

Resultados: Se estudiaron 38 pacientes donde la edad media fue de 68.21 ± 15.354 años (rango 37 a 95 años) y su procedencia fueron un 31.58 % (n=12) de Lima, 26.32% (n=10) de Piura, 10.53% (n=04) de La Libertad, 7.89% (n=03) de Arequipa, 7.89% (n=03) de San Martin, 5.26% (n=02) en Ancash e Ica y 2.63% (n=01) Lambayeque, Tumbes y Moquegua (Gráfico N° 1). En el grupo HER2/neu (+) con RE (-) con estadio clínico III o IV (n=20) con 100% (n=20) de recurrencia en 5 años, la sobrevida libre de enfermedad es de 12.600 meses con límite inferior 10.685 y límite superior 14.515 y un rango de 7 a 24 meses. Y en el grupo Luminal A (RE + y/o RP +, HER2-) con estadio clínico III o IV (n=18) tuvieron recurrencia 45.6% (n=10) en 5 años y fueron censurados 44.4% (n=8); su sobrevida libre de enfermedad de 48.556 con un límite inferior de 42.022 y límite superior de 55.089, con un rango de 16 a 60 meses.

Conclusión: La sobrevida libre de enfermedad en pacientes con HER2/neu (+) con RE (-) con estadio clínico III o IV es de 12.60 meses siendo 35.956 meses menor a la de los pacientes con Luminal A con estadio clínico III o IV que tuvieron una sobrevida libre de enfermedad de 48.556 (p< 0,001). El estudio concuerda con los resultados obtenidos con José Baselga et col (37) obtiene una sobrevida libre de enfermedad en pacientes con HER2/neu (+) RE (-) de 12.4 meses, y con Tsuda H et col (18) donde se determina una disminución de la sobrevida libre de enfermedad en pacientes HER2 (+) RE (-) a comparación de otras amplificaciones genéticas, lo cual predice mal pronóstico de los pacientes.

Objective: Specify free survival of patients with pathologic diagnosis disease infiltrating ductal carcinoma Herb2 / neu (+) ER (-) and luminal A with Clinical Stage III or IV in the Oncohematology Service Central Military Hospital between the Years 2006-2014. Material and Methods: We produce a descriptive, analytical, retrospective study of survival rate. The Study Population consisted of 38 Patients Who are asigned into two groups, one with Her2 / neu (+) ER (-) with Clinical Stage III or IV (n = 20) and the other with luminal A (n = 18) with Clinical Stage III or IV with infiltrating ductal breast carcinoma in the Service Oncohematology of the Central Military Hospital in Lima Period 2006-2014 Results: : We study a total of 38 patients, the mean age was 68.21 ± 15.354 years (range 37-95 years) and provenance 31.58% (n = 12) of Lima, 26.32 % (n = 10) of Piura, 10.53% (n = 04) of La Libertad, 7.89% (n = 03) of Arequipa, 7.89% (n = 03) of San Martín, 5.26 % (n = 02) in Ancash and Ica and 2.63% (n = 01) Lambayeque, Tumbes and Moquegua (Graphic No. 1). In the HER2 / neu (+)RE (-) group with Clinical Stage III or IV (n = 20) with 100% (n = 20) of recurrence in 5 years, the free survival was 12,600 months, with Lower limit 10,685 and Upper limit of 14,515, with a range 7 to 24 months. And the luminal A (ER + and / or PR +, HER2) with Clinical Stage III or IV (n = 18) had recurrence of 45.6% in 5 years (n = 10) and 44.4% were Censored (n = 8); its free survival is 48,556, with the lower limit of 42,022 and 55,089 Upper Limit of disease, with a range of 16-60 months. Conclusion: The disease-free survival in patients with HER2 / neu (+) ER (-)with Clinical Stage III or IV is 12.60 months being less than 35,956 months of Patients with luminal A with Clinical Stage III or IV had A free survival of 48, 556 months (P<0,001). The study is agree with the results obtained with José Baselga et col (37) gets a disease-free survival in patients with HER2 / neu (+) RE (-) of 12.4 months, and Tsuda et col (18) Where determine decreased of disease-free survival in patients HER2 (+) ER (-) a comparison of the luminal A and predicts poor prognosis of this patients.
