To assess whether the scale of modified early warning (MEWS) is more effective than the APACHE II((Acute Physiology and Chronic Health Evaluation II) in predicting severity of acute pancreatitis in the Edgardo Rebagliati Martins form Lima National Hospital during the period January 2012 to December 2014. MATERIAL AND METHODS: We conducted an observational, analytical, diagnostic test study, which evaluated 66 patients > 18 years of age, divided into two groups, group 1 (9 patients with severe acute pancreatitis) and group 2 (57 patients with mild to moderate acute pancreatitis). RESULTS: The mean age in groups 1 and 2 were 55.78 ± 18.25 and 50.37 ± 19.63 years (p > 0.05); the 55.56% and 31.58% were males in groups 1 and 2 respectively (p > 0.05). The most common etiology was biliary in both groups. The averages of the values obtained from the modified early warning score highest ( hMEWS ) within 24 hours and modified early warning score mean (mMEWS ) in 24 hours was 1 5.33 ± 0.71 vs 3 23 ± 1.02 ( p < 0.001 ) respectively and group 2 was 4.02 ± 0.36 vs 2.61 ± 0.60 ( p < 0.001 ) respectively ; the average values obtained from the APACHE II score in Group 1 was 8.33 ± 2.45 and Group 2 was 3.91 ± 2.41 ( p < 0.001 ) . Sensitivity , specificity, positive predictive value ( PPV) and negative predictive value (NPV ) were for the modified early warning score highest ( hMEWS ): 100 % , 17.54% , 16.07% and 100% respectively ; For The modified early warning score mean ( mMEWS ): 100 % , 77.19 % , 40.91 % and 100% respectively and the APACHE II was 77.78% , 92.98 % , 63.64 % , 96 , 26% respectively. The diagnostic accuracy were for the modified early warning score highest (hMEWS) in 24 hours = 0.955, modified early warning score mean (mMEWS) 24 hours = 0.979 and APACHE II = 0.890 11 CONCLUSIONS: The modified early warning highest(hMEWS) both the as he modified early warning score mean(mMEWS) in 24 hours were more effective than APACHE II(Acute Physiology and Chronic Health Evaluation II) in predicted severe acute pancreatitis.
