To determine whether the integration of ultrasound findings with Alvarado score has greater diagnostic effectiveness than isolated Alvarado score in the diagnosis of acute appendicitis (AA) in children. MATERIAL AND METHODS: We conducted an observational, analytical, diagnostic test study, which evaluated 55 patients suspected of acute appendicitis in the period of study comprised between January 2010 to December 2013. RESULTS: The mean age of the entire sample was 10.91 ± 12.47 years; the 61.22% in the AA group were male and in the group without AA were 83.33%. By associating the Alvarado score dichotomized and diagnosis of AA was observed that 91.84% had a score of Alvarado ≥ 7 and those without AA were referred positive as 33.33% (p < 0.01); when the score Alvarado dichotomized more positive ultrasonography for AA and pathological diagnosis of AA was associated, it was observed 97.96% in the group AA and had not had the disease were referred to as positive 16.67% (p < 0.001). The sensitivity, specificity, PPV and NPV of Alvarado score was only 91.84%, 66.67%, 95.74% and 50% respectively (for a score ≥ 7) and in relation to the Alvarado score more U.S.; sensitivity, specificity, PPV and NPV of 97.96%, 83.33%, 97.96% and 83.33% respectively was obtained. CONCLUSIONS: The Alvarado score more positive ultrasonography to AA was more effective in the diagnosis of acute appendicitis that the Alvarado score alone.
