OBJECTIVES: To compare morbidity and type of severe maternal morbidity among women undergoing elective cesarean delivery and vaginal delivery. MATERIALS AND METHODS: An observational, analytical, retrospective cohort study, which evaluated 460 pregnant women divided into two groups: group I (230 pregnant women undergoing elective cesarean section) and group II (230 pregnant women undergoing vaginal delivery). RESULTS: The mean age in group I was 27.92 ± 7.67 and in group II 28.91 ± 6.52 years old (p> 0.05), average parity in group I was 2, 05 ± 1.21 and in group II was 2.25 ± 1.08 (p <0.05), gestational age at delivery was 39.29 ± 1.45 average weeks in group II was 39 , 75 ± 1.71 weeks (p <0.01). With regard to severe postpartum maternal morbidity was observed that hysterectomy was present in group I at 1.30% and 0.43% in group II (p> 0.05), hemorrhagic shock in the group I 5.65% and 1.74% in group II (p <0.05), the presence of blood transfusions in group I 9.13% and 3.91% in group II (p <0.05 ) anesthetic complications were present in 2.61% group I and group II in 0% (p <0.05), puerperal infection in group I and II was 1.74% and 5.22 % respectively (p <0.05) and uterine rupture in group I and 0.43% in group II 0% (p> 0.05). CONCLUSIONS: Hemorrhagic shock, blood transfusions, anesthetic complications and puerperal infection were found to be associated with type of delivery.
