Objective: Determine the effectiveness of biological meshes compared to synthetic meshes in ventral hernia repair. Materials and methods: A systematic review was carried out, searching databases such as: SCOPUS, Web of Science, Embase and PUBMED, using a search strategy with the terms: synthetic, biologic, polypropylene, ventral, hernia, repair, matrix., creating search expressions that adapt to each proposed database. A checklist from the PRISMA 2020 statement was used. Storage and selection of studies in each phase of the review process was executed using the Rayyan web program. The evidence collected that answers the research question was analyzed and executed using Review Manager 5.4.1 software. Data were documented as risk ratio (RR) with 95% confidence interval (CI) for dichotomous outcomes. Results: After the search, application of inclusion and exclusion criteria, analysis of the complete article, information was extracted from 8 articles, the average age of patients with mesh or biological was 57 years, the female sex predominated in the majority of the studies. The total population of all selected studies was 1156 patients, of which 717 (62.6%) used synthetic mesh in the surgical intervention and 429 (37.4%) patients used biological mesh. The effectiveness was evaluated with the presence of postoperative complications such as surgical site infection, of which the biological mesh showed 17.2% (74 patients) compared to the synthetic mesh with 6.9% (50 patients) incidence. Conclusions: Biological mesh showed less effectiveness compared to synthetic mesh in patients with ventral hernia repair
