Objective: To determine if the RIPASA score has a higher diagnostic value than the modified Alvarado score for acute appendicitis at the Victor Lazarte Echegaray hospital, in the 2018 period. Material and method: retrospective, observational study of diagnostic tests, where 268 medical records of patients with preoperative diagnosis of acute appendicitis were evaluated. Results: The RIPASA test had a Sensitivity of 23.3, Specificity 94.7%, Positive Predictive Value 98.3% and Negative Predictive Value 8.6%. The modified Alvarado test, had a Sensitivity of 51.0%, Specificity 63.2%, Positive Predictive Value 94.8% and Negative Predictive Value 9.0%. When comparing both scores a highly significant statistical difference (p <0.01) with respect to Sensitivity is observed, being higher in the modified Alvarado score. The characterization of the group of patients according to age, shows that 66.0% were between 20-49 years; and only 4.9% had 70 or older. The mean age and standard deviation were 35.7 and 16.2 years respectively. Regarding sex, 57.8% were male and 42.2% female. Conclusions: The modified Alvarado score showed a higher diagnostic value than the RIPASA score in the diagnosis of acute appendicitis.
