Objective: Determine the risk factors for primary failure after the creationof an arteriovenous fistula for hemodialysis. Matherial and Method: A retrospective, analytical, case-control study was carried out in a population made up of 104 patients with chronic kidney disease who underwent an arteriovenous fistula for hemodialysis, divided into two groups: 26 patients who presented primary failure after the creation of the arteriovenous fistula (cases) and 78 who did not have such failure (controls). Results: The variables that were significantly associated with primary failure after the creation of the arteriovenous fistula for hemodialysis were: arterial hypotension (OR: 8.21, 95% CI: 4.61-41.52, p <0.000), obesity (OR: 6.91, CI 1.19-40.26 , p = 0.015), parathyroid hormone> 70 pg / ml (OR: 3.08, CI: 1.00- 9.84, p = 0.050), fistula that previously failed (OR: 66.0, CI: 7.94-548.75, p <0.000; these same variables had a significant association in the multivariate analysis except parathyroid hormone> 70 pg / ml. Variables that were not associated with primary failure after the creation of an arteriovenous fistula for hemodialysis were: age, sex, presence of diabetes mellitus, arterial hypertension, location anatomy of arteriovenous fistula, stage of chronic kidney disease and hypoalbuminemia. Conclusion: Risk factors for primary failure are hypotension, decreased diastolic blood pressure, obesity, parathyroid hormone> 70pg / ml and fistula that previously failed.
