To determine the usefulness of the Neutrophil-lymphocyte index as a predictor of early complications in patients post-operative with EB bariatric surgery at the Víctor Lazarte Echegaray and Alta complexity Virgen de la Puerta hospitals between the years 2015 and 2023. Material and methods: A retrospective cohort analytical study was carried out where 192 patients who underwent bariatric surgery and who met the inclusion criteria were included. The variables of interest were collected. Qualitative variables were evaluated using Pearson's Chi-square test with relative risk, using 95% confidence intervals. The Mann-Withney U test was used for quantitative variables. Statistical significance was developed with a value of 5% (p<0.05). Results: According to the Relative Risk (RR) test, by multivariate analysis, it was found that elevated NLR was not significantly associated with the presence of early complications (aOR = 1.71; 95% CI = 0.61 – 4. 80; p = 0.31). In addition to this, no other factor was found associated with these complications in the patients. The median age was 43 years and the median operative time was 222.5 minutes. Likewise, 64.1% were women and 35.9% were men, 40.6% suffered from high blood pressure, 33.3% had diabetes, 40.6% showed hypercholesterolemia. In relation to early complications, there were 15.1% where, of the total number of patients, none showed bleeding or deep vein thrombosis, 2.1% had pulmonary thromboembolism, 1.6% showed dehiscence, 1.0% had stenosis, 9.9% had surgical site infection (SSI) and 2.1% had urinary tract infection. Additionally, 25% showed a high neutrophil-lymphocyte ratio (NLR). Conclusion: The neutrophil-lymphocyte index was not significantly associated (aOR = 1.71; 95% CI = 0.61 – 4.80; p = 0.31) with the presence of early complications after bariatric surgery. in the Víctor Lazarte Echegaray and Alta complexity Virgen de la Puerta hospitals between 2015 and 2023
