Introduction: Nationwide, 15,192 patients suffer from comorbidity of diabetes mellitus and tuberculosis; 283 of them reside in Piura. In Peru, the accumulated cases from 2013-2019 are 9,720 patients with MDR-TB, of these 68 reside in Piura Diabetes has a significantly higher risk of developing MDR-TB, it is two or three times higher than in people without DM. These patients have more severe symptoms and are at greater risk of treatment failure and relapse. Objectives: To determine if Diabetes Mellitus as a risk factor for developing Multidrug-resistant Tuberculosis in patients older than 15 years in the department of Piura from 2013-2019. Methods: Case control study. A total of 305 patients with Pulmonary Tuberculosis over 15 years of age in Piura from 2013-2019; of which 61 patients with TB-MDR for the cases and 244 with sensitive TB for the controls, chosen with simple random sampling. Through the report in the medical records, the file was filled out. Simple regression models were also performed, estimating the Odds ratio (OR), with its corresponding 95% CI (confidence interval) and Pearson's Chi Square for their association. Results: A 68.9% of previous DM was identified in the cases and 68.9% in the controls (OR=1 (0.545 - 1.833); 95% CI; p= 1.0), in addition 78.7% of the final DM belongs to the cases and 78.7% to the controls. (OR= 1 (0.504-1.984); 95% CI; p= 1.0). The mean age was 38 years for cases and 35 years for controls. Sociodemographic variables such as sex (male, female), socioeconomic level (A, B, C, D, E); housing position (urban, rural); They did not have statistical significance, as well as the pathological background variables such as: pulmonary fibrosis, alcoholism, smoking. Nutritional status: normal, overweight, grade I, grade II, grade III presented a P=0.987; anemia with a P=0.56; Therefore, these factors have not been associated with MDR-TB. Conclusion: Diabetes Mellitus is not a risk factor for MDR-TB in patients older than 15 years in Piura from 2013-2019.
