Abstract: Determinar el impacto de la resistencia a carbapenémicos en la
mortalidad de los pacientes con diagnóstico de sepsis.
Método: Se realizó un estudio observacional, analítico, retrospectivo de cohorte,
donde se revisaron 201 historias clínicas, excluyendo 78, seleccionando 42
pacientes expuestos y 42 no expuestos a infecciones por microorganismos
resistentes a carbapenémicos que evolucionaron a sepsis en el Hospital Víctor
Lazarte Echegaray. Las historias clínicas fueron seleccionadas por criterios de
inclusión, primordialmente por el diagnóstico de sepsis y con resultados de cultivo
y antibiograma. Los datos fueron recolectados en una base de datos de Excel y se
importaron al paquete estadístico Stata v16.0 donde se hizo el cálculo de riesgo
relativo y el análisis de las variables relacionadas con la prueba de chi2
.
Resultados: La tasa de mortalidad general en la cohorte de 84 pacientes
hospitalizados por sepsis fue 67%. La mortalidad en los pacientes con infecciones
por microorganismos resistentes a carbapenémicos fue 88% y 45% en los
pacientes sin dicha resistencia, p<0.001. El riesgo relativo de mortalidad de los
pacientes con infecciones por microorganismo resistentes a carbapenémicos fue
1.51; IC 95% (1.07-2.14; p=0.020) en relación a los pacientes sin dicha resistencia.
Conclusión: La resistencia a los carbapenémicos si está asociada a un mayor
riesgo de aumento en la mortalidad de los pacientes con sepsis.

The medical files were selected by inclusion criteria, predominantly by diagnosis of sepsis and culture and antibiogram results. The data was collected in an Excel database and imported into the statistical package Stata v16.0, finally the calculation of the Relative Risk and the analysis of the variables with the chi-square test were performed. Results: The overall mortality rate in the cohort of 84 patients hospitalized for sepsis was 67%. Mortality in patients with infections due to microorganisms resistant to carbapenems was 88% and 45% in patients without such resistance, p<0.001. The relative risk of mortality of patients with infections due to microorganisms resistant to carbapenems was 1.51; 95% CI (1.07-2.14; p=0.020) in relation to patients without said resistance. Conclusion: Carbapenem-resistance is associated with a higher risk of increased mortality in patients with sepsis.
