Objective: To determine if preperitoneal transabdominal hernioplasty (TAPP) has a lower risk of complications, shorter surgical time, and shorter hospital stay than totally extraperitoneal hernioplasty (TEP) in patients older than 18 years after inguinal hernioplasty at the Hospital de Alta Complejidad “Virgen de la Puerta“ Material and methods: An analytical, cohort, retrospective study was carried out. The population consisted of 113 patients with a diagnosis of inguinal hernia who underwent preperitoneal transabdominal hernioplasty (TAPP) or totally extraperitoneal hernioplasty (TEP) at the Hospital de Alta Complejidad “Virgen de la Puerta” during the period from January 2015 to November 2019. Results: Complication rates do not show significant differences between both surgical procedures: transabdominal preperitoneal hernioplasty (TAPP) and totally extraperitoneal hernioplasty (TEP). The frequency of recurrences was 7% in TAPP and 5% in TEP, the frequency of surgical site infection was 0% in both TAPP and TEP, the frequency of chronic pain was 1% in TAPP and 5% in TEP, the hematoma frequency was 3% in TAPP and 0% in TEP, and the seroma frequency was 2% in TAPP and 0% in TEP. Conclusions: Transabdominal preperitoneal hernioplasty (TAPP) has the same risk of complications as totally extraperitoneal hernioplasty (TEP). The percentage of complications with both TEP and TAPP is low in both cases, and they are not significant.
