Objective: To demonstrate that vaginal hysterectomy is a safer approach than total abdominal hysterectomy in patients diagnosed with benign disease of the uterus not prolapsed in the HRDT during the period January 2015 - October 2018. Material and methods: A retrospective cohort study was carried out in which 110 patients with non-prolapsed benign pathology were included, according to selection criteria which were divided into 2 groups: exposed to vaginal hysterectomy or abdominal hysterectomy; applying the relative risk, and the chi square statistical test. Results: No significant differences were observed regarding the variables age, hospital stay in days and frequency of comorbidities among patients exposed to vaginal or abdominal hysterectomy. The frequency of complications was lower in the group with vaginal hysterectomy (4 %) than in total abdominal hysterectomy (13 %) with a relative risk of 0.26 which was not significant (p > 0,05). The averages of blood loss and operative time were significantly lower in the group of patients exposed to vaginal hysterectomy with respect to the group exposed to abdominal hysterectomy (p <0.05). Conclusion: Vaginal hysterectomy is a safer approach to blood loss and operative time, than total abdominal hysterectomy in patients diagnosed with benign disease of the uterus not prolapsed in the HRDT during the period January 2015 - October 2018.
