To determine the effectiveness of rectally applied diclofenac sodium in preventing acute pancreatitis in patients undergoing endoscopic retrograde cholangiopancreatography. Material and Methods: Retrospective cohort study developed with 216 patients who underwent endoscopic retrograde cholangiopancreatography, divided into two groups; 108 patients who received diclofenac sodium and 108 patients who did not receive diclofenac sodium, at the Hospital Militar Central ““Crl. Luis Arias Schreiber““. The review of the Atlanta international consensus and a data collection form for the respective study were used as a guide for the diagnosis of acute pancreatitis. Results: Of the total sample, 108 patients underwent treatment with sodium diclofenac where only 9 (8.3%) developed acute pancreatitis; Of the other 108 patients who did not undergo diclofenac sodium treatment, 24 (22.2%) developed acute pancreatitis. Of the total cases of pancreatitis, 22 (66.7%) were women and 11 (33.3%) were men, both with a mean age of 50.0 years (range, 26-67 years). Regarding comorbidities, 23 (10.6%) suffered from diabetes mellitus and used their usual medication, 27 (12.5%) suffered from arterial hypertension and used their usual medication, 7 (3.2%) suffered from cirrhosis and 13 (6.0%) patients presented obesity. Of the total number of patients, 20 (9.2%) reported smoking and 17 (7.8%) alcoholism. Only 3 patients were referred to the polyvalent intensive care unit (Polyvalent ICU). Conclusions: Rectally applied diclofenac sodium is effective in preventing acute pancreatitis post endoscopic retrograde cholangiopancreatography, being a safe option for this procedure
