Introduction: The restless legs syndrome is a neurological sensorimotor disease with an uncertain etiology that can affect quality of life and has not been deeply studied to the date. Multiple studies involve the iron supplementation as a treatment for the restless legs syndrome, however, there aren´t systematical reviews that confirm the iron deficiency as a risk factor for this disease. Objective: To determine if iron deficiency constitutes a risk factor for the developing of restless legs syndrome in patients with ages between 6 and 18 years old. Methods: A systematic review was preformed using mainly 8 online databases. The selection criteria included all the observational analytical studies which correlates iron deficiency and restless legs syndrome y patients younger than 18 years old and provides a risk coefficient, relative risk, odds ratio and confidence interval of 95% or enough data to calculate this measures. Two authors made an independent selection, data extraction and risk of bias assessment. The dichotomous data analysis with the difference of measurements and standard deviation was planned as well as with odds ratio or relative risk for continuous data. Results: A total of 1125 articles where assembled from online databases; however, only one study was included in this review. A total of 200 patients with ages between 8 and 18 years old were evaluated, 98 of which had iron deficiency or iron deficiency anemia with values of serum ferritin less than or equal to 12 ng/ml and 102 with levels of serum ferritin above 12 ng/ml; finding an incidence of Restless legs syndrome in the first group with a percentage of 61.2 %. The multivariate logistic regression analysis of the risk factors determined that iron deficiency and iron deficiency anemia are independent risk factors highly related to restless legs syndrome (OR 0.95; P= 0.000; IC 95%: 0.93 – 0.97). Furthermore, a statistically significant correlation was found between serum ferritin levels less than 50 ng/ ml and restless legs syndrome. Conclusions: A comparative statistic synthesis was not possible to make due to the lack of analytical studies whose evaluate the relationship between iron deficiency and restless legs syndrome. Thus we couldn’t stablish a relationship between this two factors. The only article included in this review stablish that de iron deficit is a significant independent factor related to Restless legs syndrome.
