Objective: The primary purpose of this study is to determine whether the reverse Shock Index by Glasgow Coma Scale (rSIG) is a reliable predictor of mortality in patients with moderate and severe TBI. Patients and Method: A case-control study was conducted, including patients admitted to the Emergency and Critical Care Department of the Regional Teaching Hospital of Trujillo (HRDT) during the period 2018 - 2023. A comprehensive analysis was performed to determine the predictive capacity of rSIG as a prognostic factor for mortality in patients diagnosed with moderate to severe traumatic brain injury (TBI). Results: The analysis revealed that rSIG is significantly associated with mortality in patients with moderate and severe TBI (ORa = 3.1; 95% CI = 1.16 – 8.7; p <0.001). Furthermore, when adjusting the prediction model for variables such as hemoglobin (Hb), anisocoria, and the reverse Shock Index (rSI), it was observed that rSIG and anisocoria remained independent factors associated with mortality. Conclusion: In our study, the product of the Reverse Shock Index and the Glasgow Coma Scale is a predictor of mortality in patients with moderate and severe TBI
