Determinate if the negative pressure wound therapy is more efficacy that the conventional therapy, in the treatment of the complicated diabetic foot ulcer in adults. Material and methods: A systematic review and meta-analysis was carried out, the screening process allowed obtaining 8 clinical trials and 4 observational studies, to compare the efficacy of negative pressure therapy versus conventional therapy. Results: The clinical trial population was 760 in total, 373 with negative pressure therapy and 387 with conventional therapy; were obtained for the outcomes, granulation tissue formation time (MD = − 1.77, 95% CI [-3.35 − -0.20], I2 = 97%, p = 0.03), ulcer depth (MD = 5.87, CI 95% [- 2.30 – 14.03], I2 = 96%, p = 0.16), amputation (RR = 0.79, 95% CI [0.56 – 1.11], I2 = 26%, p = 0.17); pain (RR = 1.41, 95% CI [1.07 – 1.85], I2 = 0%, p = 0.01), infection (RR= 0.50, 95% CI [0.31 – 0.81], I2=58%, p = 0.005), bleeding (RR=0.88, 95% CI [0.53 – 1.45], p= 0.61), hospital stay (MD = − 15.6, 95% CI [- 19.89 − - 11.31], I2=79%, p < 0.00001). The population evaluated in the observational studies was 222, with TPN it was 100 and with CT 122; were obtained for the outcomes, granulation tissue formation time (MD = −5.43, 95% CI [-7.42 − -3.43], I2=0%, p<0.00001), ulcer depth (MD= 0.40, CI 95% [- 1.66 – 2.46], p= 0.70), amputation (RR= 0.39, 95% CI [0.16 – 0.95], p=0.04), pain (RR=7.42, 95% CI [0.37 −149.73], p =0.19), infection (RR=0.39, 95% CI [0.16 – 0.95], p=0.04) and hospital stay (MD= - 10.90, 95% CI [ - 15.21 − - 6.59], p < 0.00001). Conclusions: The NPWT showed have more efficacious that CT to reduce the time of formation of granulation tissue, the hospital stay, deep of ulcer, and the risk of secondary amputations, infections and bleeding; in addiction of give better pain relief and quality life.
